OBJECTIVES: To undertake a cost-effectiveness analysis of restorative treatments for a first permanent molar with severe molar incisor hypomineralization from the perspective of the Brazilian public system. MATERIALS AND METHODS: Two models were constructed: a one-year decision tree and a ten-year Markov model, each based on a hypothetical cohort of one thousand individuals through Monte Carlo simulation. Eight restorative strategies were evaluated: high viscosity glass ionomer cement (HVGIC); encapsulated GIC; etch and rinse adhesive + composite; self-etch adhesive + composite; preformed stainless steel crown; HVGIC + etch and rinse adhesive + composite; HVGIC + self-etch adhesive + composite, and encapsulated GIC + etch and rinse adhesive + composite. Effectiveness data were sourced from the literature. Micro-costing was applied using 2022 USD market averages with a 5% variation. Incremental cost-effectiveness ratio (ICER), net monetary benefit (%NMB), and the budgetary impact were obtained. RESULTS: Cost-effective treatments included HVGIC (%NMB = 0%/ 0%), encapsulated GIC (%NMB = 19.4%/ 19.7%), and encapsulated GIC + etch and rinse adhesive + composite (%NMB = 23.4%/ 24.5%) at 1 year and 10 years, respectively. The benefit gain of encapsulated GIC + etch and rinse adhesive + composite in relation to encapsulated GIC was small when compared to the cost increase at 1 year (gain of 3.28% and increase of USD 24.26) and 10 years (gain of 4% and increase of USD 15.54). CONCLUSION: Within the horizon and perspective analyzed, the most cost-effective treatment was encapsulated GIC restoration. CLINICAL RELEVANCE: This study can provide information for decision-making.

ABSTRACT: Granulomatous slack skin (GSS) is a rare subtype of mycosis fungoides, and few cases have been known to spread to the blood, lymph nodes, or viscera. We present a case with early dissemination to the lung. A 27-year-old woman, previously healthy, presented with scattered disseminated scaly patches, associated with vulvar and intergluteal firm swelling and groin-skin induration for 1 year. She also reported mild fatigue and breathlessness on moderate exertion. The patient underwent blood tests, skin biopsies, and computed tomography scan. The skin biopsy showed a mildly atypical T-cell lymphoid infiltrate involving the dermis/hypodermis, with focal epidermotropism, associated with a granulomatous infiltrate and elastophagocytosis. The computed tomography scan revealed bilateral ground-glass lung nodular opacities. Positron emission tomography showed an increased signal in the skin and subcutis around the buttocks, inguinal and mediastinal lymph nodes, and lungs. The lung biopsy confirmed a dense T-cell infiltrate with numerous multinucleated giant cells. Subsequently, esophageal involvement was also observed following biopsy. Molecular analyses demonstrated identical T-cell clones in the skin and lung. After 6 cycles of chemotherapy/localized external radiotherapy, the patient had a partial skin response and stable lung disease. A preferred diagnosis of GSS with systemic spread was made based on clinical/histologic/molecular findings, after considering granulomatous mycosis fungoides and peripheral T-cell lymphoma, not otherwise specified. This case highlights the frequent diagnostic difficulty in distinguishing GSS from an inflammatory granulomatous dermatitis. Pulmonary and esophageal involvements are rare in GSS, and the simultaneous presentation of characteristic cutaneous GSS with systemic disease poses an additional classification challenge.

RESUMEN

CRISPR-Cas systems evolved in prokaryotes to implement a powerful antiviral immune response as a result of sequence-specific targeting by ribonucleoproteins. One of such systems consists of an RNA-guided RNA endonuclease, known as CRISPR-Cas13. In very recent years, this system is being repurposed in different ways in order to decipher and engineer gene expression programmes. Here, we discuss the functional versatility of the CRISPR-Cas13 system, which includes the ability for RNA silencing, RNA editing, RNA tracking, nucleic acid detection and translation regulation. This functional palette makes the CRISPR-Cas13 system a relevant tool in the broad field of systems and synthetic biology.

ABSTRACT: Primary cutaneous acral CD8(+) lymphoma (AL) has been accepted as primary cutaneous acral CD8-positive T-cell lymphoproliferative disorder in the revised WHO and updated WHO-EORTC lymphoma classifications. Commonly arising on the ears and comprising a clonal cytotoxic CD8 + T-cell infiltrate, almost all cases follow an indolent clinical course. A single aggressive case reported in the literature had a deletion at the CDKN2 locus at 9p21. We report an atypical CD8 + T-cell proliferation arising on the chest of an elderly man who had some similarities to AL but with a very high proliferation rate, absent p16 protein expression, and homozygous loss of the CDKN2 locus using FISH analysis. A diagnosis of peripheral T-cell lymphoma not otherwise specified (PTCL NOS) was preferred. Analyses of 4 cases of AL demonstrated often low p16 protein expression but intact CDKN2 loci. This case raises the problems of the boundaries between AL and PTCL NOS, and a possible role in the loss of p16 function in pathogenesis.

RESUMEN

We report the clinical and pathologic features of a patient with breast carcinoma, who developed clinically visible pigmented skin lesions during the course of the disease. The combination of clinical pigmentation, histological pagetoid epidermal spread, and considerable melanin pigment within tumour cells lead to a misdiagnosis of melanoma. This case provides a striking example of the ability of epidermotropic breast carcinoma to mimic melanoma. A literature review is also reported.

RESUMEN

This study aimed to describe the perception of the meaning of life and coping strategies of 19 patients with cancer at a public hospital in Brazil. This is descriptive and qualitative research that used Amedeo Giorgi's phenomenological model, and the interpretation was performed in light of Viktor Frankl's theory. Two themes have emerged: (1) the perception of the meaning of life in the experience with cancer and (2) religious coping to fight the disease. This study showed that revealing the meaning of life in the cancer experience contributes to better treatment compliance, a more favorable view of the future and better quality of life. On the other hand, religious coping is the strategy most used by patients who believe in God, showing a greater state of psychological and spiritual well-being.

RESUMEN

OBJECTIVE: to reflect on the applicability of the Medical Healing of Souls (MHS) by health professionals as a welcoming strategy in post-pandemic mental health. METHODS: a theoretical and reflective study, based on Viktor Frankl's philosophy, based on the book "The Doctor and the Soul, From Psychotherapy to Logotherapy" and scientific literature. RESULTS: the study was structured in two discursive approaches: MHS in the field of health; The applicability of MHS in post-pandemic mental health care. FINAL CONSIDERATIONS: MHS can be used in mental health care, in health emergencies, promoting a more humane performance of health professionals, facilitating the integration of inevitable suffering into a meaningful life.

Owners' understanding of dog behaviour influences dog welfare. This study aimed to investigate owners' experiences of living with dogs and perceptions of dog behaviour/behaviour change. Data from an ongoing UK/ROI longitudinal study of dogs were used. Open-ended survey data (n = 3577 comments, n = 1808 dogs) when dogs were 12/16 weeks (data combined), 6, 12, 18 and 24 months were analysed to cover the dog's puppyhood/adolescence. To evaluate the usefulness of open-ended survey questions, both quantitative textual and qualitative thematic analyses were employed. Textual analysis identified an overall positive sentiment at all timepoints; the proportion of positive: negative sentiments increased with the dog's age. Words related to 'love' were the most frequent descriptors at all but the first timepoint, when 'bite' was the most frequent descriptor. Qualitative analysis helped to identify that owners attribute dog behaviour to 'Dog's biology', 'Personality/deliberate action' and 'External influences'. Analysis of open-ended survey responses helped to identify changes in perception over time. When dogs were young, owners described problematic behaviours as 'mischievous', unintentional and context-specific. Similar behaviours shown by older dogs were seen as 'deliberate'. Both positive and negative experiences of dog ownership were identified. However, as not all respondents answered open-ended questions, the generalisability of our findings is limited.

We developed and evaluated a simple, low-cost matrix-assisted laser desorption/ionization mass spectrometry (MALDI-TOF MS)-based method for the detection of meropenem (MER) resistance in Pseudomonas aeruginosa directly from blood. A volume of 50 µL of positive-flagged blood culture (BC) was transferred in 450 µL brain heart infusion (BHI) broth to microtubes either containing MER (test) or not (control) at 25 or 50 mg/L. P. aeruginosa was categorized as resistant or susceptible on the basis of whether or not the isolates could be identified, respectively, in the presence of the antibiotic (3 h of incubation). When using BCs spiked with 99 P. aeruginosa isolates (64 MER-resistant and 35 MER-susceptible) the method correctly classified 88/99 isolates (88.9%). Correct categorization was achieved in 23/23 (100%) of P. aeruginosa isolates (17 MER-susceptible and 6 MER-resistant) from prospectively collected BCs. Our method may prove useful for early targeted or adjustment of empirical therapy in patients with P. aeruginosa bloodstream infections.

Recurrent disease outbreaks caused by different viruses, including the novel respiratory virus SARS-CoV-2, are challenging our society at a global scale; so versatile virus detection methods would enable a calculated and faster response. Here, we present a novel nucleic acid detection strategy based on CRISPR-Cas9, whose mode of action relies on strand displacement rather than on collateral catalysis, using the Streptococcus pyogenes Cas9 nuclease. Given a preamplification process, a suitable molecular beacon interacts with the ternary CRISPR complex upon targeting to produce a fluorescent signal. We show that SARS-CoV-2 DNA amplicons generated from patient samples can be detected with CRISPR-Cas9. We also show that CRISPR-Cas9 allows the simultaneous detection of different DNA amplicons with the same nuclease, either to detect different SARS-CoV-2 regions or different respiratory viruses. Furthermore, we demonstrate that engineered DNA logic circuits can process different SARS-CoV-2 signals detected by the CRISPR complexes. Collectively, this CRISPR-Cas9 R-loop usage for the molecular beacon opening (COLUMBO) platform allows a multiplexed detection in a single tube, complements the existing CRISPR-based methods, and displays diagnostic and biocomputing potential.

The ability to control protein expression at both the transcriptional and post-transcriptional levels is instrumental for the cell to integrate multiple molecular signals and then reach high operational sophistication. Although challenging, fully artificial regulations at different levels are required for boosting systems and synthetic biology. Here, we report the development of a novel framework to regulate translation by repurposing the CRISPR-Cas13 immune system, which uses an RNA-guided ribonuclease. By exploiting a cell-free expression system for prototyping gene regulatory structures, our results demonstrate that CRISPR-dCas13a ribonucleoproteins (d means catalytically dead) can be programmed to repress or activate translation initiation. The performance assessment of the engineered systems also revealed guide RNA design principles. Moreover, we show that the system can work in vivo. This development complements the ability to regulate transcription with other CRISPR-Cas systems and offers potential applications.

Objective(s): To develop a questionnaire that assesses the level of comprehension and decision-making capacity of patients with breast cancer about palliative care and advance care planning. Methods: Questionnaire items were based on the scientific literature. Delphi Consensus, a three-round survey with experts (n = 14), evaluated the relevance, clarity, and redundancies of the items. A pretest with breast cancer patients (n = 15) evaluated whether they comprehended each item and identified doubts or discomforts. Results: The initial questionnaire was composed of 38 items. After the Delphi, 18 items were restructured, six were added, and 16 were removed. In the pretest phase, all items with the survey header, guidelines, and Likert model were evaluated. All items accomplished ≥80% cut-off score and were kept as in the original version. The final version of the questionnaire have 28 itens and five domains: determination, responsibility, independence, self-knowledge, and knowledge of reality. Conclusions: This study represents the first step in the development of a questionnaire that may be used in oncology clinical practice. The main findings revealed that Delphi and pretesting increased the quality of the questionnaire, making it compelling to assess breast cancer patients' comprehension and decision-making capacity about PC and ACP.

El propósito es analizar las características de los estudios y las tendencias en las publicaciones científicas y las recomendaciones clínicas relacionadas con el COVID-19 en odontopediatría. La búsqueda electrónica se realizó en las bases de datos MEDLINE, Scopus, Web of Science Core Collection y LILACS/BVS, sin restricciones. Se incluyeron estudios que usaron términos referentes a COVID-19, odontología pediátrica y niños en el título, resumen o palabras clave. Se extrajeron datos bibliométricos y características de los estudios. Cada estudio se categorizó de acuerdo con su diseño, muestra y tema principal. Se incluyeron 49 estudios, la mayoría revisiones (n=25; 51,00%), estudios observacionales (n=23; 47,00%) y estudio clínico (n=1; 2%). Los principales temas fueron urgencias odontológicas (n=9; 18,30%), bioseguridad (n= 8; 16,30%) y teleodontología (n=6; 12,20%). Las urgencias odontológicas más reportadas fueron trauma (n=12; 63,10%), edema (n=9; 47,30%) y dolor (n=9; 47,30%). Se concluyó que se realizaron pocos estudios sobre COVID-19 y odontología pediátrica, y la mayoría de las revisiones y estudios observacionales sobre emergencias dentales se publicaron en Asia.


O objetivo é analisar as características dos estudos e tendências nas publicações científicas e recomendações clínicas relacionadas ao COVID-19 em odontopediatria. A busca eletrônica foi realizada nas bases de dados MEDLINE, Scopus, Web of Science Core Collection e LILACS/BVS, sem restrições. Foram incluídos estudos que usaram termos referentes a COVID-19, odontopediatria e crianças no título, resumo ou palavras-chave. Os dados bibliométricos e as características do estudo foram extraídos. Cada estudo foi categorizado de acordo com seu desenho, amostra e tema principal. Quarenta e nove estudos foram incluídos, sendo a maioria revisões (n=25; 51,00%), estudos observacionais (n=23; 47,00%) e estudo clínico (n=1; 2%). Os principais temas foram emergências odontológicas (n=9; 18,30%), biossegurança (n= 8; 16,30%) e teleodontologia (n=6; 12,20%). As emergências odontológicas mais relatadas foram trauma (n=12; 63,10%), edema (n=9; 47,30%) e dor (n=9; 47,30%). Concluiu-se que poucos estudos sobre COVID-19 e odontopediatria foram realizados, com a maioria das revisões e estudos observacionais sobre emergências odontológicas publicados na Ásia


The purpose is to analyze the characteristics of the studies and trends in scientific publications and the clinical recommendations related to COVID-19 in pediatric dentistry. Electronic search was performed on MEDLINE, Scopus, Web of Science Core Collection, and LILACS/VHL databases, without restrictions. Studies that used terms referring to COVID-19, pediatric dentistry, and children in the title, abstract, or keywords were included. Bibliometric data and studies' characteristics were extracted. Each study was categorized according to its design, sample, and main subject. Forty-nine studies were included, most of them reviews (n=25; 51.00%), observational studies (n=23; 47.00%), and clinical study (n=1; 2%). The main topics were dental emergencies (n=9; 18.30%), biosafety (n= 8; 16.30%) and teledentistry (n=6; 12.20%). The most reported dental emergencies were trauma (n=12; 63.10%), edema (n=9; 47.30%), and pain (n=9; 47.30%). It was concluded that few studies on COVID-19 and pediatric dentistry were conducted, and most reviews and observational studies regarding dental emergencies were published in Asia.

Resultados: el estudio se estructuró en dos enfoques discursivos: CMA en el campo de la salud; La aplicabilidad de la CMA en la atención de la salud mental pospandemia. Consideraciones finales: la CMA puede ser utilizada en la atención a la salud mental, en emergencias de salud, promoviendo una actuación más humana del profesional de la salud, facilitando la integración del sufrimiento inevitable a una vida con sentido.

Resultados: o estudo foi estruturado em dois enfoques discursivos: A CMA no campo da saúde; A aplicabilidade da CMA no acolhimento em saúde mental pós-pandemia. Considerações finais: a CMA pode ser usada na atenção em saúde mental, em emergências sanitárias, promovendo uma atuação do profissional de saúde mais humanizada, facilitando a integração do sofrimento inevitável em uma vida com sentido.

Objetivo: analisar o papel da dimensão espiritual como um recurso terapêutico na consulta de enfermagem em saúde mental no ambulatório de cardiologia. Método: abordagem qualitativa, a partir da narrativa de vida de seis pessoas em acompanhamento no ambulatório vinculado a uma universidade pública, entre março e dezembro de 2019. Adotou-se a Análise Existencial de Viktor Frankl, utilizando a entrevista não estruturada. Estudo aprovado pelo Comitê de Ética e Pesquisa. Resultados: os dados foram organizados em duas categorias: "Se Deus existe, por que fez isso comigo?", "Estratégias de Enfrentamento Espiritual". A dimensão espiritual é propiciadora de significado e sentido da vida. Desenvolve valores essenciais como a recuperação da esperança e do desejo de viver, manifestado em ações de autocuidado e adesão ao tratamento. Conclusão: a consulta em saúde mental e cardiologia propiciou um movimento de abertura para além das limitações impostas pelo adoecimento e condições de vida, levando ao engajamento num projeto repleto de sentido.


Objective: to examine the role of the spiritual dimension as a therapeutic resource in the mental health nursing interview of cardiology outpatients. Method: this qualitative study was based on life narratives collected, between March and December 2019, in unstructured interviews using Viktor Frankl's Existential Analysis, from six people monitored at an outpatient clinic linked to a public university. The study was approved by the research ethics committee. Results: the data were organized into two categories: "If God exists, why did he do this to me?" and "Spiritual Coping Strategies". The spiritual dimension fostered meaning and sense in life. It developed essential values, including restoring hope and the desire to live, which were manifest in self-care actions and adherence to treatment. Conclusion: the mental health and cardiology interview fostered an openness, beyond the limitations imposed by illness and life conditions, which led to engagement in a project full of meaning.


Objetivo: analizar el papel de la dimensión espiritual como recurso terapéutico en la consulta de enfermería de salud mental en el ambulatorio de cardiología. Método: enfoque cualitativo, a partir de la narrativa de vida de seis personas en seguimiento médico en el ambulatorio vinculado a una universidad pública entre marzo y diciembre de 2019. Se adoptó el Análisis Existencial de Viktor Frankl, utilizando la entrevista no estructurada. Estudio aprobado por el Comité de Ética en Investigación. Resultados: se organizaron los datos en dos categorías: "Si Dios existe, ¿por qué me ha hecho esto?", "Estrategias de enfrentamiento espiritual". La dimensión espiritual propicia significado y sentido a la vida. Desarrolla valores esenciales como la recuperación de la esperanza y las ganas de vivir, manifestados en acciones de autocuidado y adherencia al tratamiento. Conclusión: la consulta de salud mental y cardiología proporcionó un movimiento de apertura más allá de las limitaciones impuestas por la enfermedad y las condiciones de vida, lo que conllevó al comprometimiento a un proyecto lleno de sentido.

Although cats and dogs can live amicably, inter-species conflict can result in poor welfare. Species introduction can impact the development of the cat-dog relationship. This study aimed to identify factors associated with owner reported 'only desirable' puppy behaviour (defined as the puppy being uninterested, ignored the cat and/or interacted in a calm way) following introduction to existing household cats, and to explore perceptions of the emerging cat-dog relationship. Owner-reported data collected as part of a longitudinal study of canine health and behaviour were used. Of 4678 puppies, 26.7% lived with at least one cat. Of the 1211 puppies who had been introduced to the household cat at the time of survey completion, playing (58.9%), being overexuberant or over-excited (56.6%), and chasing (48.6%) were the most common behaviours displayed towards cats. 'Only desirable' behaviours were shown by 7.3% of puppies. Multivariable logistic regression showed early (puppies aged <12 weeks), gradual introductions and living in a multi-dog household increased the odds of 'only desirable' behaviours. Qualitative analysis revealed two styles of introductions­owner-led and pet-led. Owners who led introductions anticipated amicable relationships between pets, whilst owners who let pets introduce themselves did not. Early, gradual, owner-led introductions of puppies to household cats should be encouraged.

Introducción: La atención integral es necesaria en todas las dimensiones de la persona afectada por tumores malignos, para identificar y monitorear los impactos que experimentan los pacientes. Objetivo: Integrar los resultados de estudios referentes a la espiritualidad y la calidad de vida de las personas con neoplasia maligna. Métodos: Revisión integradora realizada en 2021 en las bases de datos: LILAS, BDENF, MEDLINE. La pregunta guía se estructuró con el acromion PIO. La estrategia de búsqueda se realizó con los descriptores "Espiritualidad", "Calidad de Vida" y "neoplasia maligna", asociado con los operadores booleanos AND y OR. Fue utilizado el diagrama de flujo PRISMA. Criterios de inclusión: disponibilidad en su totalidad, tiempo de corte 2016-2021, publicado en portugués, inglés y español y criterios de exclusión: estudios duplicados, manuales, trabajos de conclusión de curso, disertaciones, tesis, editoriales, cartas al editor, resúmenes de eventos, reseñas integradoras y notas anticipadas. Para análisis de los resultados se realizó lectura del material para analizar si se ajustan a los criterios metodológicos preestablecidos y si se asocian al objetivo de la revisión. Fueron seleccionados para la revisión de la literatura 20 artículos. Conclusiones: La sistematización y análisis de los artículos identificados sirvieron para comprender cómo se asocia la espiritualidad con la promoción del aumento de la calidad de vida en sus diversas fases y sus implicaciones en el contexto oncológico como factor relevante de forma integral y holística para contribuir al bienestar físico, psicológico y social en diferentes grupos y países(AU)


Introduction: Comprehensive care is necessary in all the dimensions of the person affected by malignant tumors, as a way to identify and monitor the impacts experienced by patients. Objective: To integrate the results of studies concerning spirituality and quality of life of people with malignant neoplasm. Methods: An integrative review was carried out in 2021 in the LILACS, BDENF and MEDLINE databases. The guiding question was structured with the IOP acronym. The search strategy was performed with the descriptors Espiritualidad [spirituality], Calidad de Vida [quality of Life] and neoplasia maligna [malignant neoplasia], associated with the Boolean operators AND and OR. The PRISMA flowchart was used. The inclusion criteria were total availability, cohort time 2016-2021, and materials published in Portuguese, English and Spanish; while the exclusion criteria were duplicate studies, manuals, course completion papers, dissertations, theses, editorials, letters to the editor, event proceedings, integrative reviews and advance notes. For the analysis of the results, the material was read to analyze whether it met the pre-established methodological criteria and whether it was associated with the objective of the review. Twenty articles were selected for the literature review. Conclusions: The systematization and analysis of the identified articles served to understand how spirituality is associated with the promotion of increased quality of life in its various phases and its implications in the oncological context as a relevant factor in an integral and holistic way for contributing to physical, psychological and social well-being in different groups and countries(AU)

Viral infections in plants threaten food security. Thus, simple and effective methods for virus detection are required to adopt early measures that can prevent virus spread. However, current methods based on the amplification of the viral genome by polymerase chain reaction (PCR) require laboratory conditions. Here, we exploited the CRISPR-Cas12a and CRISPR-Cas13a/d systems to detect three RNA viruses, namely, Tobacco mosaic virus, Tobacco etch virus, and Potato virus X, in Nicotiana benthamiana plants. We applied the CRISPR-Cas12a system to detect viral DNA amplicons generated by PCR or isothermal amplification, and we also performed a multiplexed detection in plants with mixed infections. In addition, we adapted the detection system to bypass the costly RNA purification step and to get a visible readout with lateral flow strips. Finally, we applied the CRISPR-Cas13a/d system to directly detect viral RNA, thereby avoiding the necessity of a preamplification step and obtaining a readout that scales with the viral load. These approaches allow for the performance of viral diagnostics within half an hour of leaf harvest and are hence potentially relevant for field-deployable applications.

Combined kinetic analysis of plasma SARS-CoV-2 RNAemia, Nucleocapsid (N)-antigenemia and virus-specific antibodies may help ascertain the role of antibodies in preventing virus dissemination in COVID-19 patients. We performed this analysis in a cohort of 71 consecutive critically ill COVID-19 patients (49 male; median age, 65 years) using RT-PCR assay, lateral flow immunochromatography method and receptor binding domain (RBD) and N-based immunoassays. A total of 338 plasma specimens collected at a median of 12 days after symptoms onset were available for analyses. SARS-CoV-2 RNAemia and N-antigenemia were detected in 37 and 43 specimens from 26 (36.5%) and 30 (42.2%) patients, respectively. Free RNA was the main biological form of SARS-CoV-2 found in plasma. The detection rate for both viral components was associated with viral load at the upper respiratory tract. Median time to SARS-CoV-2-RBD antibody detection was 14 days (range, 4-38) from onset of symptoms. Decreasing antibody levels were observed in parallel to increasing levels of both RNAemia and N-antigenemia, yet overall a fairly modest inverse correlation (Rho = -0.35; P < 0.001) was seen between virus RNAemia and SARS-CoV-2-RBD antibody levels. The data cast doubts on a major involvement of antibodies in virus clearance from the bloodstream within the timeframe examined.
